Thrive Participant Waiver
Fill out Thrive’s Activities Waiver ahead of time! This waiver allows parents/guardians to give permission for their students to attend and participate in Thrive, Youth-360 and/or United Way of Mat-Su activities as detailed herein.
Please note: Thrive Mat-Su and Youth 360 are programs of United Way of Mat-Su. As such, anywhere United Way of Mat-Su is referenced in this document, Thrive and Youth 360 are included.

Thrive Mat-Su centers our work around the Positive Cultural Framework.

We promote safe supportive spaces for youth by collaborating with partners like Boys and Girls Club, Mat-Su Clubhouse; “Moving Youth in Foster Care”, Onward & Upward; Bigs/Littles Program, Big Brothers, Big Sisters; Friendship Park, A Community Gathering Place.
We facilitate capacity-building community-wide by sponsoring training for agencies and community members: True North Recovery Services, Youth Thrive training community-wide; participation in Alaska Afterschool Conference; Fallen Up Ministries; participating in distributing Safe Drug Disposal Disposal Bags & Pill Pods through Project Hope, an OSMAP program; Alaska Public Health Summit.
We engage our community in prevention and healing: Hosted the 2018 Prevention Summit; Be[You] Media Campaign: 78% of Alaska Teens Don’t Drink; Bi-Annual Safe Drug Takeback Events; Be[You] Scholarships for Valley Youth.
We help youth expand positive peer and adult relationships through: Parenting Courses & Summer Camps at Sunshine Community Health Center; Annual Thrive Mat-Su Youth Retreat; Leadership training for local high schools; Burchell High School Summer School Program; Partnerships with local agencies serving youth, like CoDi (Compassionate Directions), and supporting school district behavioral health programs.
